Edgeworth Family Practice is proud to provide care and support to Veterans and their families. We understand that as veterans and family of veterans, you may face challenges that require extra support.

For Gold Card Holders with one or more chronic health conditions, and who meet eligibility criteria, we offer DVA’s Coordinated Veterans’ Care (CVC) Program to help you take control of your health. We have a dedicated Practice Nurse to help Veterans navigate the branches of DVA with a positive outcome. We work with you and your health care team to improve your overall wellbeing which in turn may help prevent unplanned hospital admissions. This is achieved through targeted care planning with regular reviews, appropriate referrals as required and through ensuring you and your family have the education and understanding of your health to make informed decisions.
